We accept most major insurances. Please call for further information.
Be aware that insurance plans change frequently. Before your appointment, please verify with your insurance company that you are a participating member in one of our accepted plans.
I’m a new patient. What should I do to get ready for my first visit?
To help shorten your waiting time, please print and bring in the completed Patient Registration Forms
Also, please bring a photo ID and your insurance information, and arrive 10-15 minutes before your scheduled appointment time.
Do you treat children, infants, and teens?
Yes. Whenever possible, a parent or guardian should accompany minors to their appointments. However, if a minor will occasionally need to visit our practice unaccompanied, we will require an authorization form signed by the parent/guardian.
How does online care work?
Click here to enter the Prizant Dermatology online waiting room. You’ll be asked to create a secure account with DermatologistOnCall, and then upload pictures of your condition. Within three business days, the dermatologist will review your case, you’ll receive a diagnosis and treatment plan and prescriptions, if necessary, will be sent to your preferred pharmacy.
